ISA 2006: enable ICMP pings from the internal network

The default ISA 2006 firewall configuration denies all traffic to and from any host. I successfully configured the OWA publishing rule for Exchange 2007, but my initial tests from the Exchange server showed that ICMP was disabled. All hosts on the internal network were prevented from using the ping command.
